2: Face – to – Face Assessment
Student has completed a face-to-face assessment with a client of an area mental health service in the company of a mental health clinician. For the purpose of this assignment the client must be de- identified.

Face-to-face assessment includes all relevant mental state examination inquiries and specific risks i.e. suicide

During the face-to-face assessment it is expected the student will be able to provide a rationale for the structure of their assessment and explain complex topics to their preceptor.

3:Psychiatric Assessment
Student has completed and documented a psychiatric assessment using the organisational template (or if not available use template provided by the university), which includes (but is not limited to):
– Presenting problem
– Medical, psychiatric and family history
– Genogram (with three generations)
– Pre morbid personality
– Mental state examination
– Risk assessment
– Alcohol and drug screening
– Incomplete areas provide a clear rationale indicating reason for omission from assessment

4:Clinical Formulation and Provisional Diagnosis
Student provides a clearly articulated clinical formulation, which includes a summary of referral source, current presentation and how the current presentation is affecting their psychological, biological and social wellbeing. Student provides a provisional diagnosis with differential diagnoses (where applicable), which is taken from either the ICD-10 or the DSM 5

5:Management Plan
Develop a management plan based on the principles of recovery by focussing on the person’s strengths, resources, skills and assets. Your plan should also include evidence based practice by conducting a comprehensive review of the literature and clinical practice guidelines related to the person’s diagnosis. The plan needs to clearly outline your role as nurse or allied health professional After developing a management plan, the student must compare their management plan to the plan currently in place for the individual and provide a written critique of any differences in the plans, including an explanation as to why there are differences.
