Description of Assessment:
Students are provided with a written detailed case study on which to write a case report that includes assessment considerations, formulation, intervention plan with reference to the appropriate literature, and a letter back to the referrer updating them on your assessment and intervention plan. Use the information below and the marking rubric to provide a guide
on what include in the sections of the report.

Assessment Considerations
In this section interpret the assessment information provided and write a brief mental status examination, either in this section, or with the formulation. You should also include in your report any ongoing assessments you would include, noting the test and at what time points
you would utilise it.

Formulation
Use the 4 P formulation. If there is any additional information that you would have liked to have known or follow up on flag it in this section. This section should also include your thoughts on diagnosis and any provisional diagnosis.

Intervention plan:
Based on the empirically based literature, detail the intervention approach you would use with this client. In this section you should also outline how you would implement the treatment goals. This should not be a session by session outline, but provide a broader description, with consideration to the initial sessions (for example psychoeducation, relaxation training), treatment focused sessions (for example a form of exposure therapy)
and termination/concluding treatment. Make sure that you provide sufficient detail in your intervention plan.
