Context:
The principles and philosophy of primary healthcare, health promotion and community development practice will be called upon in exploring the health of individuals and communities. Health professionals need to develop their capacity for empathy and understanding of the lived experience of vulnerable population groups as well as pathways for treatment and possible barriers to accessing these.
Understanding these principles and philosophies is critical in developing appropriate community nursing care. Without understanding the fundamentals underpinning any profession, it can be difficult to appreciate why procedures are delivered in the way they are.
The Australian National Health Priority Areas (NHPAs) highlight areas where primary health care is essential for improving health outcomes in the future. In addition, The National Strategic Framework for Chronic Conditions has identified nine Priority Target Populations those that are negatively impacted by chronic conditions more than the general population. This is demonstrated by a higher prevalence of chronic conditions and a greater burden of disease in these populations resulting in
inequitable health outcomes.
The information that students have within the subject material, needs to find its way to those who need it most and to the general population. This information is often presented as an educational activity conducted by community nurses. Assessment 2 Part A provides the rationale, justification and planning for this education activity and Part B is a digital presentation.
Instructions:
- View the three case scenario videos provided and found in your Assessment 2 area
- Choose one of these scenarios, Theo, Rhonda or David
- You are to justify the need for an education event tailored to meet the needs of your chosen character in the scenario.
- This event may either be in the form of an individual education session, or a group education event to a group of similar target population individuals (NOTE: Part A contains the justification and Part B the details of the event).
- To justify the need for such an event you must use evidence, as nursing is an evidence-based profession.
- Consider the logistics of your education session – duration, location, time of day, health literacy level of information and provide justification for your decisions.
- Provide statistics and refer to current guidelines where appropriate to support your discussion.
- Consider cultural and contextual differences for your character in the Australian setting.
To do this you should address the following key points:
- What is your target health condition and which NHPA/s are you addressing?
- What are the risk factors for developing the health condition?
- What are the long-term consequences of the condition? What are the health issues/risks specific to your character?
- What social determinants of health does your character experience?

Report Structure
Your report should include:
Introduction
This section requires you to provide background information on the topic you have chosen to investigate (including historical context, changes in our understanding etc.), the purpose of your report and what information your report will contain. This section is to be written in complete sentences and paragraphs. It does not to contain any dot point information or tables, graphs and diagrams. It is to provide ‘signposting’ for the reading – giving an overview of what is to follow in the report.
Body
This section will be broken up into a series of subheadings which will address each of the key areas you are required to research for your topic. The majority of these sections should be written in complete sentences and paragraphs however, you can include graphs, diagrams and tables that may
be appropriate. Ensure that you title these clearly, in the correct way, and reference using the APA referencing system where necessary. Tables, graphs and diagrams can also be added in an appendix at the end of your report, and when doing this, you need to clearly state where the item can be
located. Generally, information presented in this format does not count towards your overall word count. Some information may be presented in dot point format, however, if the majority of your information is set out in this way it makes it difficult to evaluate your level of understanding for each area being researched.

Conclusion:
This section brings together all of the information that you have presented in your report, and links back to your introduction any information that adds value to our overall understanding of the topic.You can also discuss any areas which have been identified as requiring further research or
investigation and how this will work to improve or change our understanding of the topic. This section does not introduce or discuss any new information specifically, and like the introduction, will be written in complete sentences and paragraphs. No tables, graphs, diagrams or dot points.
Appendix

This is an academic report; therefore, you must use appropriate language, spelling, grammar and punctuation. Your assessment must be submitted as a word document and not in protected view.Your assessment should be in 12-point font – Arial or Times New Roman, 1.5 line spaced and a minimum of 2.5cm margins.
